We are HIPPA compliant through our patient portal and EMR system. Our telehealth visits are HIPPA compliant as well. During your visit or pre-registration, consent forms, medical history forms and conversations containing protected health information are filled out online or within our clinic. This information can include your prescriptions, name, date of birth, address, contact information, insurance, treatments and conditions and all are kept private. Lord of Heaven's Armies is required by law to comply with these privacy terms. We are also required to notify you of any breach of your information. We use your protected health information to treat you, for payment, and health care operations. We may disclose your protected health information to pharmacies, hospitals, doctors in order to provide effective treatment and may be through the electronic health system. We may use this information to contact you for appointment reminders, test results, treatment adherence and modifications. For payments, we may share your protected health information for health insurance, payor, or third party and to contact you about payments due. We may disclose your HIPPA protected information to the individuals you have assigned prior to the visit only and to your emergency contact in cases of emergencies.
